For the integrity of FV(especially non-MemoryMapped FV) layout, let CachedFv point to FV beginning, but not (FV + FV header). And current code only handles (FwVolHeader->ExtHeaderOffset != 0) path, update code to also handle (FwVolHeader->ExtHeaderOffset == 0) path.
