cxgb4: Fix {vxlan/geneve}_port initialization

adapter->rawf_cnt was not initialized, thereby
ndo_udp_tunnel_{add/del} was returning immediately
without initializing {vxlan/geneve}_port.
Also initializes mps_encap_entry refcnt.
