This patch optimizes read command behavior when file-ns configured
with buffered I/O. Instead of offloading the buffered I/O read operations
to the worker threads, we first issue the read operation with IOCB_NOWAIT
and try and access the data from the cache. Here we only offload the
request to the worker thread and complete the request in the worker
thread context when IOCB_NOWAIT request fails.

nvmet: support for traffic based keep-alive
A controller that supports traffic based keep-alive can restart the keep
alive timer even when no keep-alive was not received in the kato period
as long as other admin or I/O commands were received. For each command
set ctrl->cmd_seen to true, and when keep-alive timer expires, if any
commands were seen, resched ka_work instead of escalating to a fatal
error.

nvme: support traffic based keep-alive
If the controller supports traffic based keep alive, we restart the keep
alive timer if any admin or io commands was completed during the kato
period. This prevents a possible starvation of keep alive commands in
the presence of heavy traffic as in such case, we already have a health
indication from the host perspective.
Only set a comp_seen indicator in case the controller supports keep
alive to minimize the overhead for pci controllers.

nvme: add a numa_node field to struct nvme_ctrl
Instead of directly poking into the struct device add a new numa_node
field to struct nvme_ctrl. This allows fabrics drivers where ctrl->dev
is a virtual device to support NUMA affinity as well.
Also expose the field as a sysfs attribute, and populate it for the
RDMA and FC transports.

nvme: consolidate memset calls in the nvme_setup_cmd path
In function nvme_setup_cmd() we call command specific setup function
for flush, rw, and discard. Instead of calling memset in each function
lets call it once in the parent function.
This is purely code cleanup patch and it does not change any existing
functionality.

blk-mq: re-build queue map in case of kdump kernel
Now almost all .map_queues() implementation based on managed irq
affinity doesn't update queue mapping and it just retrieves the
old built mapping, so if nr_hw_queues is changed, the mapping talbe
includes stale mapping. And only blk_mq_map_queues() may rebuild
the mapping talbe.
One case is that we limit .nr_hw_queues as 1 in case of kdump kernel.
However, drivers often builds queue mapping before allocating tagset
via pci_alloc_irq_vectors_affinity(), but set->nr_hw_queues can be set
as 1 in case of kdump kernel, so wrong queue mapping is used, and
kernel panic[1] is observed during booting.
This patch fixes the kernel panic triggerd on nvme by rebulding the
mapping table via blk_mq_map_queues().

block: add rq_qos_wait to rq_qos
Originally when I split out the common code from blk-wbt into rq_qos I
left the wbt_wait() where it was and simply copied and modified it
slightly to work for io-latency. However they are both basically the
same thing, and as time has gone on wbt_wait() has ended up much smarter
and kinder than it was when I copied it into io-latency, which means
io-latency has lost out on these improvements.
Since they are the same thing essentially except for a few minor things,
create rq_qos_wait() that replicates what wbt_wait() currently does with
callbacks that can be passed in for the snowflakes to do their own thing
as appropriate.

blkg reference counting now uses percpu_ref rather than atomic_t. Let's
make this consistent with css_tryget. This renames blkg_try_get to

blkcg: remove additional reference to the css
The previous patch in this series removed carrying around a pointer to
the css in blkg. However, the blkg association logic still relied on
taking a reference on the css to ensure we wouldn't fail in getting a
reference for the blkg.
Here the implicit dependency on the css is removed. The association
continues to rely on the tryget logic walking up the blkg tree. This
streamlines the three ways that association can happen: normal, swap,
and writeback.

blkcg: remove bio->bi_css and instead use bio->bi_blkg
Prior patches ensured that any bio that interacts with a request_queue
is properly associated with a blkg. This makes bio->bi_css unnecessary
as blkg maintains a reference to blkcg already.
This removes the bio field bi_css and transfers corresponding uses to
access via bi_blkg.

A prior patch in this series added blkg association to bios issued by
cgroups. There are two other paths that we want to attribute work back
to the appropriate cgroup: swap and writeback. Here we modify the way
swap tags bios to include the blkg. Writeback will be tackle in the next

blkcg: associate blkg when associating a device
Previously, blkg association was handled by controller specific code in
blk-throttle and blk-iolatency. However, because a blkg represents a
relationship between a blkcg and a request_queue, it makes sense to keep
the blkg->q and bio->bi_disk->queue consistent.
This patch moves association into the bio_set_dev macro(). This should
cover the majority of cases where the device is set/changed keeping the
two pointers consistent. Fallback code is added to
blkcg_bio_issue_check() to catch any missing paths.

dm: set the static flush bio device on demand
The next patch changes the macro bio_set_dev() to associate a bio with a
blkg based on the device set. However, dm creates a static bio to be
used as the basis for cloning empty flush bios on creation. The
bio_set_dev() call in alloc_dev() will cause problems with the next
patch adding association to bio_set_dev() because the call is before the
bdev is associated with a gendisk (bd_disk is %NULL). To get around
this, set the device on the static bio every time and use that to clone
to the other bios.

blkcg: introduce common blkg association logic
There are 3 ways blkg association can happen: association with the
current css, with the page css (swap), or from the wbc css (writeback).
This patch handles how association is done for the first case where we
are associating bsaed on the current css. If there is already a blkg
associated, the css will be reused and association will be redone as the
request_queue may have changed.

blkcg: convert blkg_lookup_create() to find closest blkg
There are several scenarios where blkg_lookup_create() can fail such as
the blkcg dying, request_queue is dying, or simply being OOM. Most
handle this by simply falling back to the q->root_blkg and calling it a
day.
This patch implements the notion of closest blkg. During
blkg_lookup_create(), if it fails to create, return the closest blkg
found or the q->root_blkg. blkg_try_get_closest() is introduced and used
during association so a bio is always attached to a blkg.

blkcg: update blkg_lookup_create() to do locking
To know when to create a blkg, the general pattern is to do a
blkg_lookup() and if that fails, lock and do the lookup again, and if
that fails finally create. It doesn't make much sense for everyone who
wants to do creation to write this themselves.
This changes blkg_lookup_create() to do locking and implement this
pattern. The old blkg_lookup_create() is renamed to
__blkg_lookup_create(). If a call site wants to do its own error
handling or already owns the queue lock, they can use
__blkg_lookup_create(). This will be used in upcoming patches.

blkcg: fix ref count issue with bio_blkcg() using task_css
The bio_blkcg() function turns out to be inconsistent and consequently
dangerous to use. The first part returns a blkcg where a reference is
owned by the bio meaning it does not need to be rcu protected. However,
the third case, the last line, is problematic:
This can race against task migration and the cgroup dying. It is also
semantically different as it must be called rcu protected and is
susceptible to failure when trying to get a reference to it.
This patch adds association ahead of calling bio_blkcg() rather than
after. This makes association a required and explicit step along the
code paths for calling bio_blkcg(). In blk-iolatency, association is
moved above the bio_blkcg() call to ensure it will not return %NULL.
BFQ uses the old bio_blkcg() function, but I do not want to address it
in this series due to the complexity. I have created a private version
documenting the inconsistency and noting not to use it.

blk-mq: remove QUEUE_FLAG_POLL from default MQ flags
We only support polling if we have poll queues now, but the flag is
being set by default. Remove the default QUEUE_FLAG_POLL setting, we'll
set it in blk_mq_init_allocated_queue() if we have poll queues available
for this device.

The ->poll_fn has been stale for a while, as a lot of places check for mq
ops. But there is no real point in it anyway, as we don't even use
the multipath code for subsystems without multiple ports, which is usually
what we do high performance I/O to. If it really becomes an issue we
should rework the nvme code to also skip the multipath code for any
private namespace, even if that could mean some trouble when rescanning.

nvme-pci: remove the CQ lock for interrupt driven queues
Now that we can't poll regular, interrupt driven I/O queues there
is almost nothing that can race with an interrupt. The only
possible other contexts polling a CQ are the error handler and
queue shutdown, and both are so far off in the slow path that
we can simply use the big hammer of disabling interrupts.
With that we can stop taking the cq_lock for normal queues.

nvme-pci: don't poll from irq context when deleting queues
This is the last place outside of nvme_irq that handles CQEs from
interrupt context, and thus is in the way of removing the cq_lock for
normal queues, and avoiding lockdep warnings on the poll queues, for
which we already take it without IRQ disabling.

nvme-pci: consolidate code for polling non-dedicated queues
We have three places that can poll for I/O completions on a normal
interrupt-enabled queue. All of them are in slow path code, so
consolidate them to a single helper that uses spin_lock_irqsave and
removes the fast path cqe_pending check.
nvme-pci: only allow polling with separate poll queues
This will allow us to simplify both the regular NVMe interrupt handler
and the upcoming aio poll code. In addition to that the separate
queues are generally a good idea for performance reasons.

Having another indirect all in the fast path doesn't really help
in our post-spectre world. Also having too many queue type is just
going to create confusion, so I'd rather manage them centrally.
Note that the queue type naming and ordering changes a bit - the
first index now is the default queue for everything not explicitly
marked, the optional ones are read and poll queues.
No one is going to poll for aio (yet), so we must clear the HIPRI
flag, as we would otherwise send it down the poll queues, where no
one will be polling for completions.

and came up with a cure for the STIPB regression:
- Provide a command line option to control the spectre v2 user space
mitigations. Default is either seccomp or prctl (if seccomp is
disabled in Kconfig). prctl allows mitigation opt-in, seccomp
enables the migitation for sandboxed processes.
- Rework the code to handle the conditional STIBP/IBPB control and
remove the now unused ptrace_may_access_sched() optimization
attempt
- Disable STIBP automatically when SMT is disabled
- Optimize the switch_to() logic to avoid MSR writes and invocations
of __switch_to_xtra().
- Make the asynchronous speculation TIF updates synchronous to
prevent stale mitigation state.

block: add cmd_flags to print_req_error
I ran into a bug where after hibernation due to incompatible
backends, the block driver returned BLK_STS_NOTSUPP, with the
current message it's hard to find out what the command flags
were. Adding req->cmd_flags help make the problem easier to
diagnose.

PCI: Fix incorrect value returned from pcie_get_speed_cap()
The macros PCI_EXP_LNKCAP_SLS_*GB are values, not bit masks. We must mask
the register and compare it against them.
This fixes errors like this:
amdgpu: [powerplay] failed to send message 261 ret is 0
when a PCIe-v3 card is plugged into a PCIe-v1 slot, because the slot is
being incorrectly reported as PCIe-v3 capable.
6cf57be0f78e, which appeared in v4.17, added pcie_get_speed_cap() with the
incorrect test of PCI_EXP_LNKCAP_SLS as a bitmask. 5d9a63304032, which
appeared in v4.19, changed amdgpu to use pcie_get_speed_cap(), so the
amdgpu bug reports below are regressions in v4.19.

MAINTAINERS: Update linux-mips mailing list address
The linux-mips.org infrastructure has been unreliable recently & nobody
with sufficient access to fix it is around to do so. As a result we're
moving away from it, and part of this is migrating our mailing list to
kernel.org.
Replace all instances of linux-mips@linux-mips.org in MAINTAINERS with
the shiny new linux-mips@vger.kernel.org address.
The new list is now being archived on kernel.org at

ocfs2: fix potential use after free
ocfs2_get_dentry() calls iput(inode) to drop the reference count of
inode, and if the reference count hits 0, inode is freed. However, in
this function, it then reads inode->i_generation, which may result in a
use after free bug. Move the put operation later.

mm/khugepaged: fix the xas_create_range() error path
collapse_shmem()'s xas_nomem() is very unlikely to fail, but it is
rightly given a failure path, so move the whole xas_create_range() block
up before __SetPageLocked(new_page): so that it does not need to
remember to unlock_page(new_page).
Add the missing mem_cgroup_cancel_charge(), and set (currently unused)
result to SCAN_FAIL rather than SCAN_SUCCEED.

collapse_shmem()'s VM_BUG_ON_PAGE(PageTransCompound) was unsafe: before
it holds page lock of the first page, racing truncation then extension
might conceivably have inserted a hugepage there already. Fail with the
SCAN_PAGE_COMPOUND result, instead of crashing (CONFIG_DEBUG_VM=y) or
otherwise mishandling the unexpected hugepage - though later we might
code up a more constructive way of handling it, with SCAN_SUCCESS.

mm/khugepaged: collapse_shmem() without freezing new_page
khugepaged's collapse_shmem() does almost all of its work, to assemble
the huge new_page from 512 scattered old pages, with the new_page's
refcount frozen to 0 (and refcounts of all old pages so far also frozen
to 0). Including shmem_getpage() to read in any which were out on swap,
memory reclaim if necessary to allocate their intermediate pages, and
copying over all the data from old to new.
Imagine the frozen refcount as a spinlock held, but without any lock
debugging to highlight the abuse: it's not good, and under serious load
heads into lockups - speculative getters of the page are not expecting
to spin while khugepaged is rescheduled.
One can get a little further under load by hacking around elsewhere; but
fortunately, freezing the new_page turns out to have been entirely
unnecessary, with no hacks needed elsewhere.
The huge new_page lock is already held throughout, and guards all its
subpages as they are brought one by one into the page cache tree; and
anything reading the data in that page, without the lock, before it has
been marked PageUptodate, would already be in the wrong. So simply
eliminate the freezing of the new_page.
Each of the old pages remains frozen with refcount 0 after it has been
replaced by a new_page subpage in the page cache tree, until they are
all unfrozen on success or failure: just as before. They could be
unfrozen sooner, but cause no problem once no longer visible to
find_get_entry(), filemap_map_pages() and other speculative lookups.

mm/khugepaged: collapse_shmem() remember to clear holes
Huge tmpfs testing reminds us that there is no __GFP_ZERO in the gfp
flags khugepaged uses to allocate a huge page - in all common cases it
would just be a waste of effort - so collapse_shmem() must remember to
clear out any holes that it instantiates.
The obvious place to do so, where they are put into the page cache tree,
is not a good choice: because interrupts are disabled there. Leave it
until further down, once success is assured, where the other pages are
copied (before setting PageUptodate).

mm/khugepaged: fix crashes due to misaccounted holes
Huge tmpfs testing on a shortish file mapped into a pmd-rounded extent
hit shmem_evict_inode()'s WARN_ON(inode->i_blocks) followed by
clear_inode()'s BUG_ON(inode->i_data.nrpages) when the file was later
closed and unlinked.
khugepaged's collapse_shmem() was forgetting to update mapping->nrpages
on the rollback path, after it had added but then needs to undo some
holes.
There is indeed an irritating asymmetry between shmem_charge(), whose
callers want it to increment nrpages after successfully accounting
blocks, and shmem_uncharge(), when __delete_from_page_cache() already
decremented nrpages itself: oh well, just add a comment on that to them
both.
And shmem_recalc_inode() is supposed to be called when the accounting is
expected to be in balance (so it can deduce from imbalance that reclaim
discarded some pages): so change shmem_charge() to update nrpages
earlier (though it's rare for the difference to matter at all).

Huge tmpfs testing, on 32-bit kernel with lockdep enabled, showed that
__split_huge_page() was using i_size_read() while holding the irq-safe
lru_lock and page tree lock, but the 32-bit i_size_read() uses an
irq-unsafe seqlock which should not be nested inside them.
Instead, read the i_size earlier in split_huge_page_to_list(), and pass
the end offset down to __split_huge_page(): all while holding head page
lock, which is enough to prevent truncation of that extent before the
page tree lock has been taken.

mm/huge_memory: splitting set mapping+index before unfreeze
Huge tmpfs stress testing has occasionally hit shmem_undo_range()'s
VM_BUG_ON_PAGE(page_to_pgoff(page) != index, page).
Move the setting of mapping and index up before the page_ref_unfreeze()
in __split_huge_page_tail() to fix this: so that a page cache lookup
cannot get a reference while the tail's mapping and index are unstable.
In fact, might as well move them up before the smp_wmb(): I don't see an
actual need for that, but if I'm missing something, this way round is
safer than the other, and no less efficient.
You might argue that VM_BUG_ON_PAGE(page_to_pgoff(page) != index, page) is
misplaced, and should be left until after the trylock_page(); but left as
is has not crashed since, and gives more stringent assurance.

mm/huge_memory: rename freeze_page() to unmap_page()
The term "freeze" is used in several ways in the kernel, and in mm it
has the particular meaning of forcing page refcount temporarily to 0.
freeze_page() is just too confusing a name for a function that unmaps a
page: rename it unmap_page(), and rename unfreeze_page() remap_page().
Went to change the mention of freeze_page() added later in mm/rmap.c,
but found it to be incorrect: ordinary page reclaim reaches there too;
but the substance of the comment still seems correct, so edit it down.

psi: make disabling/enabling easier for vendor kernels
Mel Gorman reports a hackbench regression with psi that would prohibit
shipping the suse kernel with it default-enabled, but he'd still like
users to be able to opt in at little to no cost to others.
With the current combination of CONFIG_PSI and the psi_disabled bool set
from the commandline, this is a challenge. Do the following things to
make it easier:
1. Add a config option CONFIG_PSI_DEFAULT_DISABLED that allows distros
to enable CONFIG_PSI in their kernel but leave the feature disabled
unless a user requests it at boot-time.
To avoid double negatives, rename psi_disabled= to psi=.
2. Make psi_disabled a static branch to eliminate any branch costs
when the feature is disabled.

userfaultfd: shmem: UFFDIO_COPY: set the page dirty if VM_WRITE is not set
Set the page dirty if VM_WRITE is not set because in such case the pte
won't be marked dirty and the page would be reclaimed without writepage
(i.e. swapout in the shmem case).
This was found by source review. Most apps (certainly including QEMU)
only use UFFDIO_COPY on PROT_READ|PROT_WRITE mappings or the app can't
modify the memory in the first place. This is for correctness and it
could help the non cooperative use case to avoid unexpected data loss.

userfaultfd: shmem: add i_size checks
With MAP_SHARED: recheck the i_size after taking the PT lock, to
serialize against truncate with the PT lock. Delete the page from the
pagecache if the i_size_read check fails.
With MAP_PRIVATE: check the i_size after the PT lock before mapping
anonymous memory or zeropages into the MAP_PRIVATE shmem mapping.
A mostly irrelevant cleanup: like we do the delete_from_page_cache()
pagecache removal after dropping the PT lock, the PT lock is a spinlock
so drop it before the sleepable page lock.

userfaultfd: shmem/hugetlbfs: only allow to register VM_MAYWRITE vmas
After the VMA to register the uffd onto is found, check that it has
VM_MAYWRITE set before allowing registration. This way we inherit all
common code checks before allowing to fill file holes in shmem and
hugetlbfs with UFFDIO_COPY.
The userfaultfd memory model is not applicable for readonly files unless
it's a MAP_PRIVATE.

userfaultfd: shmem: allocate anonymous memory for MAP_PRIVATE shmem
Userfaultfd did not create private memory when UFFDIO_COPY was invoked
on a MAP_PRIVATE shmem mapping. Instead it wrote to the shmem file,
even when that had not been opened for writing. Though, fortunately,
that could only happen where there was a hole in the file.
Fix the shmem-backed implementation of UFFDIO_COPY to create private
memory for MAP_PRIVATE mappings. The hugetlbfs-backed implementation
was already correct.
This change is visible to userland, if userfaultfd has been used in
unintended ways: so it introduces a small risk of incompatibility, but
is necessary in order to respect file permissions.
An app that uses UFFDIO_COPY for anything like postcopy live migration
won't notice the difference, and in fact it'll run faster because there
will be no copy-on-write and memory waste in the tmpfs pagecache
anymore.
Userfaults on MAP_PRIVATE shmem keep triggering only on file holes like
before.
The real zeropage can also be built on a MAP_PRIVATE shmem mapping
through UFFDIO_ZEROPAGE and that's safe because the zeropage pte is
never dirty, in turn even an mprotect upgrading the vma permission from
PROT_READ to PROT_READ|PROT_WRITE won't make the zeropage pte writable.

userfaultfd: use ENOENT instead of EFAULT if the atomic copy user fails
Patch series "userfaultfd shmem updates".
Jann found two bugs in the userfaultfd shmem MAP_SHARED backend: the
lack of the VM_MAYWRITE check and the lack of i_size checks.
Then looking into the above we also fixed the MAP_PRIVATE case.
Hugh by source review also found a data loss source if UFFDIO_COPY is
used on shmem MAP_SHARED PROT_READ mappings (the production usages
incidentally run with PROT_READ|PROT_WRITE, so the data loss couldn't
happen in those production usages like with QEMU).
The whole patchset is marked for stable.
We verified QEMU postcopy live migration with guest running on shmem
MAP_PRIVATE run as well as before after the fix of shmem MAP_PRIVATE.
Regardless if it's shmem or hugetlbfs or MAP_PRIVATE or MAP_SHARED, QEMU
unconditionally invokes a punch hole if the guest mapping is filebacked
and a MADV_DONTNEED too (needed to get rid of the MAP_PRIVATE COWs and
for the anon backend).

We internally used EFAULT to communicate with the caller, switch to
ENOENT, so EFAULT can be used as a non internal retval.

mm/page_alloc.c: fix calculation of pgdat->nr_zones
init_currently_empty_zone() will adjust pgdat->nr_zones and set it to
'zone_idx(zone) + 1' unconditionally. This is correct in the normal
case, while not exact in hot-plug situation.
After this, node1 will have its nr_zones equals to (ZONE_NORMAL + 1)
instead of (ZONE_MOVABLE + 1).

If all pages are deleted from the mapping by memory reclaim and also
moved to the cleancache:

(nrpages becomes 0)
We don't clean the cleancache for an inode after final file truncation
(removal).
truncate_inode_pages_final
check (nrpages || nrexceptional) is false
no truncate_inode_pages
no cleancache_invalidate_inode(mapping)
These way when reading the new file created with same inode we may get
these trash leftover pages from cleancache and see wrong data instead of
the contents of the new file.
Fix it by always doing truncate_inode_pages which is already ready for
nrpages == 0 && nrexceptional == 0 case and just invalidates inode.

As backtrace shows above, ocfs2_reserve_clusters() will call inode_lock
against the global bitmap if local allocator has not sufficient cluters.
Once global bitmap could meet the demand, ocfs2_reserve_cluster will
return success with global bitmap locked.
After ocfs2_reserve_cluster(), if truncate log is full,
__ocfs2_flush_truncate_log() will definitely fall into deadlock because
it needs to inode_lock global bitmap, which has already been locked.
To fix this bug, we could remove from
ocfs2_lock_allocators_move_extents() the code which intends to lock
global allocator, and put the removed code after
__ocfs2_flush_truncate_log().
ocfs2_lock_allocators_move_extents() is referred by 2 places, one is
here, the other does not need the data allocator context, which means
this patch does not affect the caller so far.

mm/gup: finish consolidating error handling
Commit df06b37ffe5a ("mm/gup: cache dev_pagemap while pinning pages")
attempted to operate on each page that get_user_pages had retrieved. In
order to do that, it created a common exit point from the routine.
However, one case was missed, which this patch fixes up.
Also, there was still an unnecessary shadow declaration (with a
different type) of the "ret" variable, which this patch removes.
Keith's description of the situation is:
This also fixes a potentially leaked dev_pagemap reference count if a
failure occurs when an iteration crosses a vma boundary. I don't think
it's normal to have different vma's on a users mapped zone device
memory, but good to fix anyway.

sbitmap: optimize wakeup check
Even if we have no waiters on any of the sbitmap_queue wait states, we
still have to loop every entry to check. We do this for every IO, so
the cost adds up.
Shift a bit of the cost to the slow path, when we actually have waiters.
Wrap prepare_to_wait_exclusive() and finish_wait(), so we can maintain
an internal count of how many are currently active. Then we can simply
check this count in sbq_wake_ptr() and not have to loop if we don't
have any sleepers.
Convert the two users of sbitmap with waiting, blk-mq-tag and iSCSI.

sbitmap: ammortize cost of clearing bits
sbitmap maintains a set of words that we use to set and clear bits, with
each bit representing a tag for blk-mq. Even though we spread the bits
out and maintain a hint cache, one particular bit allocated will end up
being cleared in the exact same spot.
This introduces batched clearing of bits. Instead of clearing a given
bit, the same bit is set in a cleared/free mask instead. If we fail
allocating a bit from a given word, then we check the free mask, and
batch move those cleared bits at that time. This trades 64 atomic bitops
for 2 cmpxchg().
In a threaded poll test case, half the overhead of getting and clearing
tags is removed with this change. On another poll test case with a
single thread, performance is unchanged.

also a hyperv bugfix, and the usual binder bugfixes.
All of these have been in linux-next with no reported issues"
