man: Canonicalize .TH usage

man: Canonicalize .TH usage

* Use all caps for document title.
* Remove section name as it can be inferred from the section number.
* Name "OpenZFS" as the document source.
* Bump modification date.

While here, fixed trailing whitespace reported by igor.
